Desert Dolphin Application Instructions

• Click on the Application form below. A little box with an arrow will appear at the top left, right about the form. Click that little box, which will download the application form to your browser.

• Click the Print Icon to print out the application.

• Fill out hard-copy Application - make sure you fill out both pages.

• Initial the Indemnification Clause and Sign the document.
